How You’ll Make a Difference

Reporting to the Integrated Health Manager, the role of Diabetes Educator provides a visiting service to all Sunrise clinics and communities providing individual and group services to diabetic patients and families. This clinical role will see you providing high-quality and individualised diabetes management/care for patients and their families suffering from, or at risk of, diabetes and associated conditions. 

Your day-to-day duties will include but not be limited to: 

  • Working collaboratively with all health center and specialist staff to ensure appropriate planning and implementation of care for patients receiving care for their diabetes and associated conditions;
  • Providing clinical leadership to other health professionals, including Remote Area Nurses (RANs), Aboriginal Health Practitioners, and medical and allied health professional staff in the area of diabetes management;
  • Managing relevant Medical Benefits Scheme claims for care provided as required and applicable; and
  • Promoting a multidisciplinary team approach to decision-making and care provided.

What You'll Need to Succeed:

The ideal candidate for this position will have:

  • A qualified health professional who either holds or is eligible for, registration with AHPRA
  • You must also hold existing accreditation as a Diabetes Educator and demonstrated professional experience in diabetes and chronic disease management.
  • Current unrestricted manual driver's License.
  • Non-adverse National Criminal History Check.
  • Current OCHRE clearance.
  • Rights to work in Australia 
  • Highly developed communication and interpersonal skills.
  • Ability to work independently under limited direction, prioritise workload, and manage time effectively.
